ASSUMPTION OF RISK
I understand that being on the premises of Impact Dance Center may involve inherent risks, whether I or my child am observing activities, participating in events, or using the facilities. I voluntarily assume all risks associated with my or my child's presence on campus.
RELEASE AND WAIVER OF LIABILITY
In consideration of the opportunity to visit and use the facilities of Impact Dance Center, I, on behalf of myself or my child, hereby release, discharge, and hold harmless Impact Dance Center, its owners, directors, employees, agents, and all associated individuals from any and all liability, claims, demands, actions, or causes of action that may arise from injuries or damages associated with our visit, whether related to participation in activities or simply being on the premises. This waiver includes claims of negligence on the part of the studio, its employees, or its agents.
ACKNOWLEDGEMENT OF POLICIES
I acknowledge that I will abide by all studio rules and policies during my visit and understand that Impact Dance Center reserves the right to ask any visitor to leave the premises for failure to comply with safety guidelines or behavior expectations.
